                      Requirements for FOSA Conversion

                                  1.  Latest deceased buried

                                  2.  Affidavit of Adjudication of Immediate Heirs

                                  3.  Waiver of Rights with Third party liability if not be named to all heirs

                                  4.  Proof of Relationship

                                        <->  Birth certificate to all children and heirs

                                        <->  Death certificate, if dead

                                        <->  Marriage Contract of husband or wife of deceased

                                  5.  Valid I.D.'s unexpired (SSS, TIN, Drivers License, Company ID, PRC ID)must be provided with signature
